APPLICATION PROCEDURES

 

The following items are required as part of the Application Process to the DPT Program at WKU.  Your application will not be considered until all of the following are received.  WKU Doctor of Physical Therapy Program has chosen to receive applications   and most applicant materials through the American Physical Therapy Association's - Physical Therapist Centralized Application Service (PTCAS)

All application materials and fees must be submitted, according to the instructions below, within the application period of July 2013 - November 1, 2013 to be considered for the Summer 2014 Program.

WKU Supplemental Requirements:

 
November 15, 2013 is the extended deadline for all Supplemental Application materials. 

 

Graduate School Application and Fee 

    1. Complete TOPNET:  WKU Admission Application - Online Application and application fee.  If you do not have a WKU ID, you will need to create a WKU web application ID.
      • Your WKU ID will be created one business day following the submission/processing of your TOPNET: Graduate School Application. 
      You will be required to place your WKU ID on all correspondence and materials sent directly to Western Kentucky University

Transcripts

    1.  Official hard copy transcripts from all previous college and professional schools must be sent directly to The Graduate School

DPT Program Technical Standards Disclosure

    1. Complete the DPT Program Technical Standards Disclosure Form indicating your awareness of and possession of a minimal level of skills and abilities necessary for Physical Therapists to practice safely and competently. 

Prerequisite Course Work

    1. The Prerequisite Course Work Form is required to document the fulfillment of both the required prerequisites and having attained GPA eligibility (SAMPLE FORM).  Conversion rates for Quarter to Semester Credit Hours are available here. 

An Interview may be required to complete the DPT Program application.
