Employee of the Month Nomination


Facilities Management Employee of the Month
Nomination Form

To be eligible for nomination:
  • Full time Employee for a minimum of at least 3 months
  • No Disciplinary Actions within the past 12 months
  • Has not been Employee of the Month in the last 12 months

 

CRITERIA's

* Met or exceeded all quality of work standards for the work performed during the month

*Attendance; clocked in and out properly every day this month including lunch, if applicable.  Volunteered when asked to work extra time to meet urgent situations, events and operational commitments.  No call outs during the month for regular shifts and "draft" events.

*Safety; not involved in any safety incidents this month, involving injuries to the individual or another crew member on the same task and/or damage to any equipment, property or material.  Safety programs training attended and notable effort and/or contribution made towards executing the safety prorgam and also maintaining and improving the DFM Safety Culture.

*Can-Do attitude, goes the extra mile in giving to the organization and doing their best
